• Team Tick List (PDF - 134kb) - a useful form for team leaders to use in order to check that each team member has had their application accepted, been disclosure checked, child protection trained etc
• Application Form (4 page PDF - 332kb) It is required that all who go on an impact team must complete this form and return it to their team leader or the iMPACT Co-ordinator. Once accepted, this form is valid until December 31st of the same year.
